Follow these simple steps to get an MTS connection:

Step 1: Fill in the Customer Application Form (CAF)

Step 2: Get your documents ready

Your Passport-sized Photograph that has been self-attested
Please carry a copy of your Proof of Identification (POI) and Proof of Address (POA):

1. For identity/Address Proof
   1. Passport
   2. Arms License
   3. Driving license
   4. Photo Identity card having address
   5. Election Commission ID Card
   6. Any Other document containing photograph includes
      1. Ration Card with Photo & address application for person applicable for person whose photo is affixed.
      2. CGHS/ECHS Card
      3. Certificate of address having photo, issued by MP/ MLA/ Group A Gazatted officer in letter head.
      4. Certified of address with photo from Govt. recognized educational institution (For student only).
      5. Certificate of address having photo issued be village panchayat head (for rural areas)
2. For identity Proof (Only)
   1. Income Tax PAN
   2. Photo Credit Card
3. For Address Proof (Only)
   1. Ration card
   2. Water Bill (not older than last 3 months)
   3. Telephone Bill of Fixed line (not older than last 3 month)
   4. Electricity Bill of State company (not older than last 3 month)
   5. Income Tax assessment Order.
   6. Vehicle Registration Certificate.
   7. Registered Sale /lease Agreement.

Note: For verification of identity/address proof, either document listed in SI. No. 1 or documents listed in SI No. 2 & 3 (One each for identity & address proof) should be obtained along with Customer Application Form before providing telephone connection.

Above all document should be in the name of Applicant only.
